SMS Terms of Service
By providing your mobile number and opting in to receive SMS communications from Integrative Wellness Center (“we,” “us,” or “our”), you agree to these SMS Terms of Service, our general Terms of Use, and our Privacy Policy.
1. Program Description
We may use SMS text messages to communicate important healthcare-related information including, but not limited to:
- Appointment reminders
- Health and wellness tips
- Care coordination and follow-up messages
- Medication or prescription refill reminders
- Billing and insurance notifications
- Patient satisfaction surveys
- Notifications related to your treatment or care plan
2. HIPAA Authorization to Disclose PHI via SMS
By opting in, you expressly authorize Integrative Wellness Center to send certain types of your Protected Health Information (PHI) to you via SMS, which may include:
- The name of your provider or facility
- Appointment dates and times
- General descriptions of your condition, treatment, or diagnosis
- Prescription names and instructions
This communication may not be encrypted, and standard SMS is not a secure method of transmitting sensitive information. You acknowledge the risks (e.g., interception, misdelivery, or access by others with access to your phone) and accept full responsibility for receiving such messages. You may revoke this authorization at any time by replying “STOP” or contacting us directly at info@mywellhealth.org or 614-321-7856.
3. User Consent
By opting in, you consent to receive SMS messages using automated or manual technology. Your consent is not required to receive care or treatment.
4. Message Frequency
You may receive recurring messages based on your appointments, treatment plans, or preferences. Frequency may vary.
5. Opt - Out
To stop receiving SMS messages, reply “STOP.” You may receive a final confirmation message. Opting out will not impact your access to care.
